The place of tracheostomy in tetanus neonatorum.

چکیده
It has been shown that total curarization and intermittent positive pressure respiration (IPPR) improves the prognosis of newborn babies suffering from tetanus (Wright, Sykes, Jackson, Mann, and Adams, 1961), but facilities for its use are not always available. The physician responsible for treating a patient may have to turn to simpler means such as tracheostomy. Some consider this essential (Creech, Woodhall, and Ochsner, 1950; Smythe and Bull, 1961), while others have failed to show any advantage from its use (Wright, 1960; Veronesi, 1956), and many have pointed out its dangers (Liston, 1844; Durham, 1869; Reading, 1949, 1958; HughJones, 1959; Smythe, 1963; Plum and Dunning, 1956; Stothers, 1956; Forbes, Salmon, and Herweg, 1947). In the investigation reported here an attempt is made to evaluate the place of tracheostomy in neonatal tetanus.

Assisted respiration in the treatment of neonatal tetanus.
Conservative management of severe tetanus neonatorum is attended by a very high mortality (Tompkins, 1958; Wright, 1960a; Adams, Holloway, and Thambiran, 1964). Tracheostomy in addition to sedation has not been successful in preventing respiratory failure, which necessitated mechanical ventilation (IPPV) in about 90% of the patients described by Holloway (1967). Although IPPV has been shown to ...

Intermittent positive-pressure ventilation and muscle relaxants were first used in Cape Town in 1958 in an attempt to reduce the mortality from tetanus neonatorum, which was then over 90%. Problems of effective ventilation, of tracheostomy, and of infection in the neonate were gradually overcome so that between 1967 and 1972 the mortality in 186 cases was 21%. In a consecutive series of 97 case...
متن کاملClostridium tetani infections in newborn infants: a tetanus neonatorum review.
Although tetanus is a preventable disease by vaccination, it continues to claim lives around the world. Whereas cases of accidental origin reflect insufficient population immunization, tetanus neonatorum reveals a double-nature fault-poor vaccination coverage of adults coupled with difficulties accessing appropriate prenatal care; this situation is aggravated by the extreme severity of tetanus ...

Waar dit onmoontlik is en die bevalling tuis plaasvind behoort dit deur ’n geneesheer of distrik...

cases led to a great decrease in the post mortems on tetanus cases. In an analysis of 1,600 post mortems performed by me in Calcutta, recoicle in my Findlayson lectures of 1925, I showed that 2.05 per cent of the total deaths were due ? to tetanus; compared with only 0.1 per cen , twenty times less, in 1,000 post mortems at a London hospital. I also quoted Calcutta s vital statistics to show th...
